ORAL SURGERY - IMMEDIATE DENTAL IMPLANTS TORONTO - IMPLANT CROWN
Mr. T. came to my clinic with a huge tooth cavity on his central incisor, multiple other teeth cavities and missing premolar teeth. The central incisor looked like a broken tooth, and structurally, there was nothing to hold on to.

If the situation isn't that invasive, the solution would be a root canal treatment, core build-up with a fibre post and an all-ceramic dental crown. Unfortunately, the patient was far away from a tooth-supported solution. Periodontally, the patient didn't have any significant gum disease.
​
The treatment I recommended was to extract the tooth, place an immediate dental implant, fill the extraction socket with bone graft, and finish the treatment with an implant-supported crown with zirconia abutment. This way, I can provide a permanent and long-term solution for his chief concern.

We started with taking intra-oral pictures and dental impressions. This step is crucial for our treatment. With this approach, we can plan and visualize the outcome of our treatment and prepare temporary prosthodontics for the healing phase of the dental implants. Eating, drinking or smiling shouldn't be an issue during our treatment phase. No patient should leave our clinic without a tooth in their mouths.

After obtaining a temporary Maryland bridge from the dental lab, I could start the treatment. The first thing that I needed to achieve was to extract the tooth root with an a-traumatic approach because after an extraction, we expect bone resorption on the buccal bone plate. If we can provide minimally invasive implant surgery and bone graft to the extraction socket, the outcome can be impeccable.

Although a single tooth implant is a considerably easy implant surgery, providing this treatment in the anterior zone of the mouth makes the procedure challenging. Therefore, we want both cosmetically perfect and functionally successful treatment in the anterior area of the mouth.

The permanent crown must match the adjacent teeth' colour and shape. The implant-supported crown needs to look exactly like natural teeth. The tooth colour was yellowish, but the patient didn't have any concerns about the shade.

After placing the dental implant and bone grafting the extraction socket, I cemented the Maryland bridge looks exactly like a natural tooth. During the osseointegration phase of the dental implant, he used this bridge as a temporary tooth replacement.

After a five to six months healing period, I requested dental x-rays to assess the healing of the bone around the dental implant. If we don't see any marginal bone loss around the dental implant, we assume that bone healing is ideal, and we can proceed with the prosthodontics phase of the treatment.

Following the assessment X-Rays, I took the impression of the implant and shade pictures. Next, I requested zirconia custom abutment for the final all-ceramic dental crown from my dental lab. My dental lab uses state-of-art dental CAD/CAM technologies to fabricate dental abutments and crowns.

After receiving the implant-supported dental crown, I inserted the crown with a try-in paste and assessed the implant-supported crown with my patient. My patient loved the outcome and accepted the insertion of the crown. Next, I used permanent cement to insert the implant-supported dental crown.

Replacing a tooth with an implant is our last resort. Our goal as Toronto Dentists is to keep the natural teeth as long as possible before extracting and replacing them with dental implants. But if we have a hopeless tooth, trying to keep it in the mouth could be a waste of money and time. Instead, we need to weigh the advantages and disadvantages of every possible treatment and pick a long-term solution.
