Miss. C. came for a consult after her Invisalign is over. Her chief concerns were the gaps between her teeth, surface stains on her lower front teeth.

She wasn't satisfied with the result of her orthodontic treatment, but the orthodontist provided the best treatment that she can. Unfortunately, not every case can be resolved by only orthodontic treatment or only restorative treatments alone. Most of the cases require different approaches of different dental specialties.
​
In some cases, we need to use only orthodontic treatments to align the teeth. In these cases, patients don't have major morphological issues or size discrepancies on their teeth. Aligning them can resolve most of their chief concerns.

On the other hand, some patients with misaligned teeth see proportional size issues and morphological problems that can't be treated by orthodontic treatments alone. In these cases, we need to use the orthodontic treatment for relocating teeth to their ideal positions. Then, after completing the orthodontic treatment, we need to reshape their teeth with restorative dentistry.

Miss C.'s teeth were aligned ideally, but she has short teeth, a gummy smile and teeth gaps because of the size discrepancies. Also, she has extrinsic stains on her lower dental bondings and morphological issues on her lower lateral incisors.
​
On patients like Miss C., we need to approach our patients with a multidisciplinary dentistry approach. We need to plan their treatment with a comprehensive spectrum.

Since she finished her orthodontic treatment, I had no control over the location of her lower and upper front teeth. However, her oral health was in good condition. I recommended comprehensive treatment planning for her. If we can design, plan and show the outcome of our treatment to the patient, it becomes easy for them to understand the recommendations we provide.

After designing and her treatment, I sent the documents and impressions of her teeth to the lab. They transferred my 2D design to 3D digital wax-up and printed her new prototype smile. With this model in my hand, I could transfer her new smile to her mouth using temporary crown and veneer materials.

We call this transfer application "Mock-Up". With this approach, we can show our patients the possible outcome of our treatments before starting the treatment. With the smile design protocol, we can show the improvement we can provide.

In her smile makeover treatment, I recommended laser gum contouring with osteotomy, teeth whitening on the teeth that weren't covered by restorative therapies. After three to four months of healing for the crown lengthening, I recommended six to ten veneers on her upper front teeth, and four lower front stained teeth four dental crowns.

As dental professionals, we need to assess every possible angle when we are planning treatment. This is because we are changing everything permanently, and we need to make sure every treatment we provided is minimally invasive and necessary.

Being necessary can be a subjective issue. However, as a cosmetic dentist, I can say that not having a confident, healthy and beautiful smile is a psychological issue that ruins patients' lives. Therefore, having confidence in your appearance is significantly important for many of our patients.

Miss C. accepted my treatment plan. We started her treatment with digitally-guided gum reshaping. Since we needed to reshape her bone level under her gum line, we needed to wait for three to four months for the stability of soft tissues.

While we were waiting, we provided teeth whitening with whitening products. For efficient teeth whitening, we need to pay attention to what we eat, drink, or smoke for 14-21 days. In this period, we should follow a strict "white diet".

After the waiting period for the soft tissue healing was over, we started the restorative phase of our treatment plan. She wanted to start with six veneers, and if needed, she wanted to continue covering the premolars on the upper teeth with dental veneers. For the lower front teeth, to cover the diastemas and change the discoloured teeth, I provided all-ceramic dental crowns. Both dental veneers and dental crowns are perfectly resistant to wear and tear.

Permanent crowns and veneers were fabricated using CAD/CAM digital dentistry production techniques in 14-days. I used rubber-dam isolation for the delivery of the veneers and dental crowns. Two weeks after insertion, I took the final pictures and videos of the patient.

Gum tissue needs to fill between the restoration more. To have that, we need to wait a little more. Also, it would be nice to add the premolars to complete the size adjustment. Who knows? Maybe we can add four dental veneers on the upper soon.
